External and internal causes of impaired skin integrity

A

Alteration that occurs in the epidermis or dermis (or both) of the skin. External causes include allergens, chemicals, cold and heat extremes, excretions or secretions, pressure, radiation, and shearing forces. Internal causes include medications, impaired circulation, impaired oxygen transport, altered metabolic state, infection, and emaciation.

Indications of impaired skin integrity include damaged or destroyed tissues; color changes such as pallor, flushing, or discoloration; altered sensations such as numbness, tingling, burning, tenderness, or pain; and edema.
