Describe the mechanism of action of proton pump inhibitors.

A

Irreversibly inactivates H+/K+/ATPase, inhibiting acid secretion for up to 24hrs (time taken for synthesis of new enzymes).

Why is an IV infusion preferred to a single bolus dose?

A

PPIs only inactivate actively secreting acid pumps, hence a single dose will only decrease acid secretion for a few hours.

Regarding oral formulations of proton pump inhibitors, please describe strategies used to increase their bioavailability and activity.

A

Taken as inactive pro-drugs

Acid-resistant enteric coating to prevent gastric elimination

Take on an empty stomach as food decreases the bioavailability

Take one hour prior to a meal so that peak plasma levels occur when most pumps are active
