accidental exposure to organophosphates in insecticides, or nerve agents

effect of organophosphate poisoning
A
increased muscarinic activity, CNS excitation (seizures), depolarising neuromuscular block; highly lipid soluble so easily accesses systemic circulation

treatment of organophosphate poisoning
A
atropine (iv) to block muscarinic receptors, artificial respiration, pralidoxime (iv) to act at nicotinic and muscarinic receptors to rescue AChE from block (reverse block within first couple of hours, otherwise irreversible)
