Epi 1:10
0.01 mg/kg
Epi 1:1
0.01 mg/kg (IM/SQ) with a max single dose of 0.3mg
Amiodarone
5 mg/kg IVP cardiac arrest
5 mg/kg over 20 min with a pulse
Lidocaine 2%
1 mg/kg for a dysrhythmia
0.5 mg/kg (max 40mg) for io anesthetic
Lidocaine maintenance drip
20-50 mcg/kg/min for a dysrhythmia
Mag sulfate
25-50 mg/kg for Torsades de Pointes
Adenosine
0.1 mg/kg then 0.2 mg/kg (SVT)
Atropine
0.02 mg/kg for Bradycardia
Defibrillation
2 J/kg - 4 J/kg for lethal dysrhythmia
Dopamine
10-20 mcg/kg/min (start at 10) - hypotension
Albuterol
2.5 mg in 3 mL normal saline (nebulizer) - respiratory distress/ anaphylaxis
Synchronized cardioversion
- 5 J/kg - 1.0 J/kg - 2.0 J/kg
- Tachycardic dysrhythmia

5 mL/kg (max single dose 100 mL)
All hypoglycemic pediatric patients
Narcan
0.1 mg/kg with a max dose of 2mg
Opioid overdose
Normal saline
20 mL/kg
-hypotension
Versed (midazolam)
- 1 mg/kg
- procedural sedation/ seizures/ post intubation
Benadryl (Diphenhydramine)
1 mg/kg
-respiratory distress/ anaphylaxis
Calcium chloride
20 mg/kg
-hyperkalemia and TCA overdose
Etomidate
- 3 mg/kg - RSI/ Intubation
0. 1 mg/kg - procedural sedation/ post intubation
Glucagon
- 5 - 1 mg/kg
- hypoglycemia or beta blocker overdose
Ketamine
2 mg/kg in a 50 mL bag with a 60 gtts wide open
- RSI/ Intubation
Sodium Bicarb 8.4%
Patients > 1 month
1 meq/kg
-TCA OD and Hyperkalemia
Valium (Diazepam)
- 2 mg/kg or 0.5 mg/kg Rectal
- Seizures/ Sedation
