Fever Reduced tone Poor feeding Resp distress Jaundice within 24 hours

What are red flags for neonatal sepsis?
A
Confirmed/suspected sepsis in mother
Signs of shock
Seizures
Term baby needing ventilation
Resp distress starting >4 hours after birth
Presumed sepsis in other baby in multiple pregnancy

How is suspected neonatal sepsis investigated?
A
FBC
CRP
Blood cultures
+/- lumbar puncture if meningitis suspected

How is neonatal sepsis managed?
A
Benzylpenicillin + gentamicin (for 7-10 days)
Fluids/feeds

When can you consider stopping abx in neonatal sepsis?
A
36 hours
- blood culture negative
- clinical condition is good
- CRP going down
