Pharm and Actions
Analgesic; Peripheral vasodilator; Pupil constriction; Respiratory depressant; Cardiac effects of vasodilation- decreases myocardial oxygen consumption, deacreses left ventricular end- diastolic pressure; decreases cardiac work load; may decreases incidence of dysrrhythimas
Indications
Chest pain not relieved by NTG; Pulmonary edema; Extremity fractures in absence of any head, chest, or abdominal injuries; Back/neck injuries when sedation/ pain relief are necessary to prevent a patient from moving around and potentially injuring themselves
Contraindications
Hypotension; Head or abdominal injuries; Patients with respiratory difficulties except for pulmonary edema; Major blood loss; Decreased level of consciousness
Administration
Adult 2-20 mg IV/IM titrate to response; Pediatric 0.1-0.2 mg/kg IV/IO
Side Effects and Special Notes
Respiratory depression; Decreased BP; Decreased level of consciousness; Decreased heart rate; N/V; Have naloxone ready to reverse administration; Allergic reactions; may follow administration with Zofran for nausea
