- Breeder and bet to complete certificate
- Medical reason or working dogs only
- Vet must receive evidence that pups meet conditions
- declaration that pup will be future working dog
- WITHIN 5 DAYS of birth
- Pup needs to be microchipped before 8 weeks at the same practice
