irritable bowel syndrome
most common GI disorder that affects the large intestine and its motor and sensory Fx in the gut
etiology
- unclear w/ proposed links to: FODMAPs, emotional stress, and cigarette smoking
FODMAPs
carbohydrates that the small intestine poorly absorbs and causes digestive distress when consumed
pathology theories
1) FODMAPs causes gas build-up in the large intestine (relates to symptoms of IBS)
2) serotonin not signaling properly –> inc peristalsis causing pain
serotonin
a neurotransmitter that mediates perfusion, motility, secretion, and sensation of pain in the gut
mnfts
- dyspepsia (abd pain)
- alternating pattern of diarrhea and constipation
- mucus in stools
- flatulence (gas trapped)
dx
- process by exclusion (testing)
- labs: Abs, CBC, stool exam, protozoan parasites
- gastroscopy, colonoscopy
- clinical presentation
tx
- avoid/eliminate offending foods in diet
- dec stress levels
- drugs prn
drugs used for tx
- antispasmodics
- antidiarrheals, laxatives
- Abx*
why Abx for IBS
keeps normal flora in check to see if there is any alleviation in symptoms
caution with use of Abx
it eliminates normal flora which allows pathogenic flora to cause inc harm
