Cholelithiasis Flashcards
cholelithiasis
precipitation of bile content –> gall stones
etiology
- genetic predisposition
- altered bile composition (abn bile components)
- bile stasis
how does an altered bile composition cause cholelithiasis
inc precipitate in bile –> stones to form
ex. high cholesterol in bile precipitates
how does bile stasis cause cholelithiasis
obstruction in the bile pathway (ex. constricted ducts) –> bile precipitates –> stones form
patho
nucleus forms from inflammatory debris, cellular components –> precipitation goes around the nucleus –> stones develop
stone types
- cholesterol stones
- pigment stones
- mixed stones
cholesterol stones composition
bile and excess cholesterol
pigment stones composition
bilirubin and Ca2+ salts
mixed stones composition
cholesterol, bilirubin, phosphate, Ca2+
mnfts
- asympt until stone migrates into bile duct
- if stone migrates into bile duct, it causes: pain + N/V
what pain occurs
intermittent colicky pain that radiates from flank to groin and chest
why is the pain intermittent
d/t stone’s migration path
dx
- Hx, Px
- U/S
- CT scan
tx
- pain meds
- dissolving agents to break up stone
- Sx to remove stones
