Why does BP fall during early pregnancy?
Vasodilation
-lowest at 22-24 weeks
What is pregnancy induced hypertension?
High BP that develops in the 2nd half of pregnancy and resolves within 6 weeks of delivery
CNS disease related to high BP
Eclampsia Hypertensive encephalopathy Intracranial haemorrhage Cerebral Oedema Cortical Blindness Cranial Nerve Palsy
Renal disease related to high BP
Decreased GFR Proteinuria Increased serum uric acid (also placental ischaemia) Increased creatinine / potassium / urea Oliguria /anuria Acute renal failure acute tubular necrosis renal cortical necrosis
Liver disease related to increased BP
Epigastric/ RUQ pain
Abnormal liver enzymes
Hepatic capsule rupture
HELLP Syndrome
Biggest risk factor for PIH/PET?
Previous PET (7X)
Test to predict pre-eclampsia?
Maternal uterine artery doppler at 20-24 weeks
Eclampsia management
IV labetalol /hydralazine
For seizures: magnesium sulphate
Post-partum: give aspirin (safe, cheap, thought to help prevent future (pre)eclampsia
Protein levels for pre-eclampsia diagnosis
> 0.3 g/L
or >0.3g in 24 hours
