- Narcotic agonist/analgesic

Mechanism of Action (MOA)?
A
Fentanyl Citrate MOA:
- Elevates pain threshold by acting on pain receptors in the brain
- Depresses CNS, depresses brainstem respiratory centers, decreases responsiveness to changes in PaCO2
- Vasodilation (venous pooling) thereby reducing reload and afterload

Indications?
A
Fentanyl Citrate Indications:
- Analgesia (especially burns, trauma, myocardial infarction, renal colic)
- Sedation in post-intubation state
- Adjunct to induction in rapid sequence induction/intubation (RSI)

Contraindicatons?
A
Fentanyl Citrate Contraindications:
- Know allergy or intolerance
- Elevated intracranial pressure
- Head injury with ALOC (relative)
- Asthma (relative)
- Abdominal pain (relative)

Adverse Reactions?
A
Fentanyl Citrate Adverse Reactions:
- CV: bradydysrhythmias (common), hypotension, tachydysrhythmias
- Resp: respiratory depression
- CNS: excess sedation, seizures, pupillary constriction
- GI: nausea and vomiting
- Derm: histamine release may cause local or generalized urticartial (hives)

Adult Dose?
A
Fentanyl Citrate Adult Dose:
- IV/IO/IM/IN: 25 - 50 mcg increments
- IV: slow push (over 1 - 5 minutes)
- Total Dose: Not to exceed 200 mcg

Pediatric Dose?
A
Fentanyl Citrate Pediatric Dose:
- 12 months to 17 years of age IV/IO/IM/IN: 1 - 2 mcg/kg increments
- IV: slow push (over 1 - 5 minutes)
- Total Dose: Not to exceed 50 mcg

A
Fentanyl Citrate Peak Effects:
- Within 5 - 15 minutes when given IV/IO/IN, Less predictable when given IM.
