Anti-phospholipid Abs: anti-cardiolipin and lupus anticoagulant

Features
A
- CLOTS: venous (e.g. DVT) and arterial (e.g. stroke)
- Coagulation defect: increase APTT
- Livido reticularis
- Obstetric complications: recurrent 1st trimester abortion
- Thrombocytopaenia

Treatment
A
Low dose aspirin
Warfarin if recurrent thrombosis: INR 3.5

Management
A
in specialist SLE and lupus nephritis clinics

Sever flares: Acute SLE
A
AIHA, nephritis, pericarditis or CNS disease
High-dose prednisolone and IV cyclophosphamide

Cutaneous symptoms
A
Treat with topical steroids and prevent with sun cream

Maintenance
A
For joint and skin symptoms
NSAIDS and hydroxychloroquine and lose dose steroids

Lupus Nephritis
A
Proteinuria:ACEi
Aggressive GN: immunosuppression
