Exam 2 Part 7 Flashcards
The effectiveness of the _____ will dictate the success or failure of therapy
Evaluation
What body parts do we examine during a medical examination?
Head and neck region, oral/nasal spaces, palpation
What do you always need in order to start voice therapy?
Physician’s approval (laryngologist)
What are the 6 steps of a voice evaluation?
Medical examination, patient interview, perceptual evaluation of voice, patient self-evaluation, instrumental evaluation of voice, functional evaluation of vocal fold movement
What parts of a patient interview should we include?
Environmental conditions, case history, medical conditions
Clinicians perform perceptual analysis using different scales like GRBAS
Perceptual evaluation of voice
Tells us how the patient evaluates their voice and how it affects QOL
Patient self-evaluation
What are examples of acoustic measures of voice?
PRAAT, MDVP
What are examples of aerodynamic measures of voice?
PAS, GE aeroview
What are examples of functional evaluations of vocal fold movement?
Laryngeal endoscopy, high-speed, kymography
What acoustic measures do we look at?
Fundamental frequency, intensity
What are the primary objectives of voice evaluation?
Understand what is causing the problem, describe present vocal components, determine how the voce disorder impacts respiration, phonation, resonance, develop an individualized plan
What are the secondary objectives of a voice evaluation?
Patient education, motivation, establish credibility of SLP
What are the parts of a voice evaluation?
Patient interview, perceptual voice assessment, instrumental assessment, laryngeal videostroboscopy
Where do referrals come from?
ENT, SLP, vocal coaches, singing teachers, family, friends
What are examples of non-speech phonotrauma?
Throat clearing, coughing, unusual laugh
Probe for the best voice possible (keeping in mind of patient’s current vocal behaviors).
Diagnostic probes
Trial diagnostic therapy and can be done through reading, singing, chanting, or counting while shaping and experimenting with alternative methods of voice production; even use of negative practice
Voice stimulability testing
Eexcessive and / or dysregulated activity of intrinsic and extrinsic laryngeal muscles, but this tension may extend to pharyngeal constrictor muscles and the deep muscles of the neck.
Paralaryngeal (PL) tension
What are symptoms of PL tension?
Dull to severe ache and tightness, increased vocal effort and fatigue, episodic neck swelling, lumps, ear fullness, restricted ROM in jaw, tongue, and respiratory movements
