Gonorrhea
Ceftriaxone 25 to 50 mg/kg/day IV or IM in a single daily dose for 7 days wduration of 10-14 days
Cefotaxime
Chlamydia
20 mg/kg azithromycin tid for 3 days
Erythromycin
Zithromax
Syphilis
Aqueous crystalline Penicillin G 100,00-150,000 units/kg/day
50,000 units/kg/dose IV every 12 hours during 1st 7 days of life or every 8 hours if infant is over 1 week of age for 10 days
Herpes
Acyclovir 20 mg/kg IV every 8 hours for 21 days (desseminated)
60 mg/kg/day for CNS disease
CMV
If gut is not working: Gangcliclovir 6 mg/kg/
If gut is working: Valganciclovir 16 mg/kg/dose every 12 hours for 6 months
Hearing screens
Hepatitis B
Interferon >1 year
lamivudine > 3 years
adefovir > 12 years
Hepatitis B perinatal transmission prevention
HBIG .5 ml 1 vaccination w/in 1st 12 hours of life. Second dose within 1-2 months and third dose at 6 months. Test for HBsAg and anti-HBs at 9 to 18 months
HIV newborn exposure
ARV prophylaxis
Zidovudine within 6 to 12 hours
Toxoplasmosis
Prevention is best. Tell pregnant women to wash fruits and veggies. Wear gloves and do not change cat litter.
Congenital infection=Pyrimethamine with Sulfadiazine with folic acid for 1 year. Consult ID
Newborn sepsis
Ampicillin and gentamycin
Dysmenorrhea
NSAIDS
OCP for 3-6 months
Omega 3 fatty acids
Endocarditis Prophylaxis
Amoxil 50 mg/kg one hour prior to dental care
Varicella
Benadryl/oatmeal bath
Acyclovir in high risk patients
Localized impetigo
Bactroban/ muciprocin/altabax
MRSA soft tissue infection
Bactrim or clindamycin
