diabeteic foot sepsis Flashcards
what do diabetic foot problems encompass
diabetic neuropathy, peripheral vascular disease and infection
what do these lead to
ulceration, necrosis and gangrene
what may it require
amputation
what is the single strongest factor for limb loss
diabetes
what may diabetic foot sepsis result from
puncture wound
infection from nail plate or interdigital space
neuroischaemic ulcer
why is diabetic foot sepsis a problem
the muscles on the foot are confined in rigid compartments, infection cant get out of them and the pus builds up and cant escape
CF
pyrexia tachypnoea tachycardia confused from sepsis kussmauls breathing
local CF
- Swollen affected digit
- Swollen forefoot
- Tenderness, pain
- Ulcer with pus extruding
- Erythema, may track up limb
- Patches of rapidly developing necrosis.
- Crepitus in the soft tissues of the foot – gas in the soft tissue from gas forming organisms.
foot pulses
absent or present
management
vascular surgical emergency
- remove infected tissue and leave wound open to encourage drainage
what is diabetic foot sepsis fundamentally a problem of
pressure
