A 75 year old lady on warfarin for atrial fibrillation is admitted with an acute upper GI bleed. Her haemaglobin is 108g/l and her INR is 7.6. She is given fluid resuscitation and 10mg intravenous vitamin K.
prothrombin complex concentrate
A 54 year old lady with chronic liver disease is listed for an endoscopic retrograde cholangio-pancreatography.
She has a platelet count of 102x109/L, a prothromin time of 28s (normal range 9-12s) and an APTT of 41s (normal range 22-30s). Her coagulation has not improved with intravenous vitamin K.

Esophagogastroduodenoscopy (OGD) and duodenum (D2) biopsy
A 67 year old woman presents with cough, fever and mild confusion. Temp is 38oC. BP 97/57mmhg, Pulse 110b/min, RR 24, Blood Tests Show HB 16gd/l, WCC 16, Plt435, Na 146,K+ 4.2, Urea 9, Creatinine 109. She has no known allergies.
V Co-amoxiclav and IV Clarithromycin
An 87 year old man presents with a right sided weakness and dysarthria. He lay on the floor for 1 day before admission. On examination he has a temperature of 39oC and has right basal crackles. SPO2 92% on air, RR is 26, Pulse is 85b/min, BP 102/55mmhg. Urea is 16, Creatinine is 116.
This is suggestive of severe Aspiration Pneumonia and should be treated with IV Amoxicillin, Gentamicin and Metronidazole

Bruton’s agammaglobulinemia
A 28 year old man presents with a severe pneumonia that does not respond to standard antibiotic therapy. A CT scan shows multiple small gas filled cavities in both lung apices and bilateral ground-glass opacification. His
condition improves with high dose co-trimoxazole.
