Which anti-tussives are locally acting and which are centrally acting
Local: honey, menthol, camphor (throat)
Centrally: DM, codeine, DH, dph (cough centre)
What is DM medication for
For dry coughs. In order to suppress cough
What is guafenesin medication for
To loosen things, for wet cough. Assists with production to rid from chest, no suppression
Is DM and guafenisin combo good idea?
No, combo works against each other, one suppress other produces
How effective are DM and codeine
Not much larger than effect of placebo, but are best option
How fast do DM and codeine work
DM works faster, codeine needs to be metabolized
-codeine is prodrug for morphine
DM: 15-30 min
Codeine: 1-2 hours
How is codeine affected by race
Caucasian metabolize better, get reasonable amount. ultrametabolizers get alot and poor get little
S/e of codeine and DM. Drug interactions
Codeine: nausea, constipation, dizzy, pruritis
DM: nausea, little s/e
Both interact with SSRIs
-DM put out all serotonin at once. causing serotonin syndrome
Are cough suppressants safe for those with lung disorder
Concern, DM will not worsen but codeine will
What age can we not recommend cough suppresants
Under 6 years recommend nothing. Lung disorders, SSRIs
What is a normal dose of codeine
10mg. Concern when low in combo product is too high concentration of other ingredients.
Is honey beneficial in compraison to DM?
In some it performed better, good alternative
Why no honey in baby first year
bautulism via raw honey. After 1 get antibodies to deal with it. Pasturized prob safe
Effectiveness of camphor/menthol
Work locally at the back of your throat, does not suppress the cough. Menthol and DM are okay combo
Effectiveness of diphenhydramine
eg. benadryl
-1st gen sedative antihistamine, reduces coughing. does nothing for actual cough but provides sedation.
How does Guaifenesin work as expectorant
Stimulates esophagous, enduces goblet cells, producing more respiratory fluid to loosen things
-best for congested/non-productive make more productive
Do we receommend CC medication under 6

What is the indication of antihistamines
decrease sneezing and rhinorrhea
How does 1st gen antihistamine work
get in brain stem and can sedate, helps with sleeping. colanergic and anti-colanergic receptors
What benefit does 2nd gen antihistamine provide
Decongestant
Lozenges and sprays
mentol
phenol/dyclonine/benzocaine
antiseptic
-spray hard for back of throat
-DM removed from so not used too much
Is zinc good for lozenges
usually sub-therapeutic. Over use is loss sense of smell
Echinacea use
Minimal benefit claim to stimulate immune system. safe
Pregnancy and CC medicine
1st trimester avoid.
Diabetes and hypertension avoidance
avoid oral decongestants
