- Enoxaparin (Lovenox)
- Dalteparin (Fragmin)
Which doses of Xarelto have to take with food
If equal to or greater than 15mg
INR over 10 w/o bleeding
Give oral vit K 2.5-5mg
If major bleed on warfarin
Give vit K 5-10mg slow IV
Treatment for pts with mechanical heart valves
Highest clot risk
Only treat with warfarin
CHADS-VASC
CHF HTN Age greater than or equal to 75 (2 pts) DM Stroke/TIA hx (2 pts)
Vasc dx (MI, PAD, aortic plaque)
Age 65-74
Sex F
Anticoagulation in pregnancy
LMWHs and monitoring anti-Xa levels recommended
