Solid tumors and some hematologic malignancies

Antitumor Doxorubicin (Adriamycin) is for
Treat for neuroblastoma, and solid tumors of bone, bladder, breast, ovary, and lung.

Sulfamethoxazole (Septra)
is for
UTI
chronic bronchitis
otitis media
MOA Sulfamethoxazole (Septra)
Bacteriostatic by Inhibiting folic acid synthesis in bacteria but doesn’t hurt human because we get it from food
Sulfamethoxazole (Septra) Nursing Concerns:
- widespread use has led to resistant strains
- Patient’s need to drink 2 liters of fluid daily
- Patients should avoid sunlight and wear sunscreen/protective clothing if necessary, to be in sun
- Can reach high concentrations in the kidneys
Sulfamethoxazole (Septra) Adverse effects:
Crystalluria
Photosensitivity (common)
Blood abnormalities
Nausea, vomiting, anorexia
Tricyclic Glycopeptides
Vancomycin (Vancocin)
Vancomycin (Vancocin)
is for
MRSA
C. diff
Staphylococcal/Streptococcal infections
Vancomycin (Vancocin)
Adverse Effects:
Ototoxicity
Aminoglycoside
Gentamicin & Tobramycin
Gentamicin & Tobramycin is used for
- Good for gram negative and sever staphylococcal infections
- Mostly used for TB & given w/ other antibiotics parenterally
Gentamicin & Tobramycin Nursing Concerns:
- Post-antibiotic effect: activity of drug continues even after serum drug levels drops
- Nephrotoxicity (renal failure)
- -Check UA for urinary casts and proteinuria
- -Check increased serum BUN and Creatinine
- Ototoxicity
- (8th cranial nerve damage) including auditory & vestibular damage
- Dizziness (Vertigo)
- Hearing loss
- Tinnitus
- -Monitor peak and trough levels due to narrow therapeutic window (toxic level close to effective level)
