Magnesium Sulphate 2 g in 50ml NS over 5 min (25gtts/15sec)

Consider analgesia/sedation
Cardioversion @ 100J, 200J, 300J, 360J

Defibrillate @ 360J
Adult VT - Monomorphic stable treatment with transport > 20 min
Mandatory OLMC - Amiodarone 150 mg in 250 ml D5W over 10 min with inline filter

Transport with pre-notification

Myocardial ischemia Hypoxia Electrolytes Acid-base imbalances Increased sympathetic tone Wolff-Parkinson-White Idiopathic causes
Adult VT - Unstable criteria
Altered level of consciousness Systolic BP less than 80 mmHg Ischemic chest pain Significant SOB CHF
Adult VT - Amiodarone indication
Stable VT with OLMC
Adult VT - Amiodarone dose
150 mg IV/IO in 250 mL D5W bag over 10 minutes with 10 gtt set and in-line filter at 4 drops per second
Do not repeat dose

Adult VT - Amiodarone EMS contraindications
Hypersensitivity to amiodarone or iodine Cardiogenic shock Sinus bradycardia 2nd or 3rd degree AV block unless a pacemaker is present Patients predisposed to intracranial hypertension Acute hepatitis Thyroid dysfunction Interstitial pulmonary disease
Adult VT - Amiodarone precautions
Use ported blunt to draw up Use caution if renal failure present Do not exceed 30 mg/min Dilute prior to administration Observe for signs of pulmonary toxicity
Adult VT - S/S of Amiodarone induced pulmonary toxicity
Progressive apnea
Cough
Fever
Pleuritic pain

Stable polymorphic VT
Adult VT - Magnesium Sulphate dose
2 g IV/IO in 50 mL NS over 5 min
