> 15 years CHF COPD RR > 25/min SpO2 < 94%
Adult Bronchospasm - CPAP contraindications?
BP < 100 mmHg
Vomiting
Possible pneumothorax
Unable to maintain airway
Adult Bronchospasm - Fluid administration in acute bronchospasm?
500 mL IV/IO bolus followed by a 250 mL/hr maintenance drip
Adult Bronchospasm - salbutamol dose?
1 puff MDI with spacer 30-60 seconds prn to a maximum of 30 puffs
5 mg neb continuously
Adult Bronchospasm - salbutamol contraindications?
Hypersensitivity
Tachydysrhythmias
Adult Bronchospasm - salbutamol MDI + spacer administration?
Prime canister 3 times
1 puff into spacer
5 breaths by patient
Adult Bronchospasm - ipratropium bromide dose?
1 puff MDI with spacer 30-60 seconds total max of 15 puffs MDI
500 mcg nebulized q 5 minutes 1500 mcg

Hypersensitivity
Adult Bronchospasm - ipratropium bromide MDI + spacer administration?
Prime canister 3 times
1 puff into spacer
5 breaths by patient
Adult Bronchospasm - epiNEPHrine (1mg/mL) indications?
Bronchospasm with impending respiratory failure
Adult Bronchospasm - epiNEPHrine (1mg/mL) dose?
0.3 mg IM q 20 min total max of 0.9 mg

None when used in emergency situations
Hypersensitivity
Underlying cardiovascular disease

Administer IM injections in the lateral quad
All patients receiving epiNEPHrine must be cardiac-monitored

50 mg PO

Patients with severe shortness of breath
Difficulty swallowing
Patients already taking prednisone
Patients with pneumonia and any symptoms of SIRS

2 – 4 hours

10 mg IM/SIVP/IO/PO
Adult Bronchospasm - dexamethasone contraindications?
Systemic fungal infections
Hypersensitivity to dexamethasone or other steroids
Hypersensitivity to benzyl alcohol or sodium sulfite
Patients with pneumonia and any symptoms of SIRS
Adult Bronchospasm - dexamethasone onset?
2 – 4 hours
Adult Bronchospasm - magnesium sulfate indications?
Severe bronchospasm refractory to nebulized medications
Adult Bronchospasm - magnesium sulfate dose?
2 g IV/IO in 50 mL NS over 10 min
Adult Bronchospasm - magnesium sulfate contraindications?
Any degree of heart block
Renal failure
