Daily Reports
A
- All unit commanders shall be responsible for reporting on a daily basis all major incidents and other
activities of interest. Division commanders shall prescribe the format for daily reports. - Time Sheets - Prepared at the beginning of each shift by supervisors to record each employee’s hours
worked and distributed to Payroll. - Neighborhood Policing Bureau shift supervisors shall ensure the preparation and submission of the
following daily reports:
a. Work Lists - Prepared by the shift supervisor reflecting officers’ work assignments. It is saved in the
appropriate folder on the server and accessible to communications personnel, the zone lieutenant,
sector captain, assistant chief of Uniformed Services Command. [11.4.1e]
b. Daily Synopsis - Prepared at the end of each shift by the shift supervisor and/or watch commander
to note any exceptional or significant activity occurring on that shift. Saved in the appropriate folder on
the server, it is accessible to the zone lieutenant, sector captain, assistant chief of Uniformed Services
Command and the PIO.

Weekly Reports
A
All commands shall submit a weekly report to the deputy chief and the Chief of Police (or their designee),
as specified by the CPD Pre-CIP meeting schedule. This report will contain a summary of significant
events within the preceding week.

Monthly Reports
A
- Vehicle Inspection Report - This report indicates the condition of each assigned vehicle and the
condition of each pool vehicle. It is prepared by the officer’s supervisor and distributed through the chain-
of-command to the appropriate assistant chief and the Facilities/Fleet Manager. - The monthly reports shall be due on the 5th working day of the following month.
- Financial reports shall be submitted in accordance with SOP-01

Annual Reports
A
- Assistant Chiefs shall submit an annual review of each specialized assignment within their respective
command to the Chief of Police as required by PER-10 Allocation and Distribution of Personnel. - Assistant Chiefs shall submit a personnel allocation/workload assessment, through the deputy chief, to
the Chief of Police in accordance with PER-10 Allocation and Distribution of Personnel. - The Departmental Goals and Objectives Report shall be submitted in accordance with ADM-11 Goals
and Objectives.
