2. Contact the City Safety Officer and request that s/he make the appropriate L&I or OSHA notification
3. If the City Safety Officer is not able to make the notification within the required timeframe, contact the local L&I office at 253-596-3800 or OSHA at 1-800-321-6742 within eight hours of the accident,
4. Provide the following information:
   a. Your name and “Tacoma Fire Department”
   b. Location and time of the accident
   c. Number of fatalities or hospitalized employees
   d. Name and phone number of a contact person for TFD
   e. A brief description of the accident
