- Assisting with investigating serious injuries or fatalities as necessary and in accordance with the guidelines set forth in this document
- Assisting L&I investigators as necessary

Who shall be responsible for making the appropriate notifications when a TFD employee is injured in accordance with the guidelines set forth in this document?

The Financial Assistant for Payroll at TFD Headquarters
Who shall report by telephone or in person immediately to the City Safety Officer when a fatality, probable fatality or inpatient hospitalization of any TFD employee occurs as a result of a work-related accident a. This includes those actually admitted, but not necessarily those who go to the hospital for precautionary measures and are released without being admitted
The TFD Administrative Battalion Chief for Safety and/or Deputy Chiefs
b. If possible, the City Safety Officer shall make the L&I notification
If the City Safety Officer is not available to make the notification, the TFD Administrative Battalion Chief for Safety and/or Deputy Chiefs shall report the fatality, probably fatality or inpatient hospitalization by telephone or in person to the nearest L&I office within 8 hours of the occurrence.
This notification requirement also applies to any subsequent fatality or hospitalization of any employee that occurs within ______ days of the accident.

If TFD does not learn of a reportable accident at the time it occurs and the accident would otherwise be reportable, the Administrative Battalion Chief for Safety or Deputy Chief shall verbally notify L & I within ____________.

In addition to L & I, the Administrative Battalion Chief for Safety or Deputy Chief shall contact the following agencies as necessary to assist with the investigation by providing expertise with documentation, record keeping and collection of evidence:

b. City of Tacoma Safety Officer
c. Tacoma Police Department
Procedure forImmediate Supervisor reporting:
- Contact Fire Communications and request a serious injury or fatality notification
- Complete HRE 002.
- Forward HRE 002 to the Financial Assistant for Payroll at TFD Headquarters.
- Enter injury in the daybook of the company where the injury occurred.
- Refer to other TFD policies that may contain additional reporting requirements related to the circumstances of the employee’s OJI.
Procedure for reporting by Fire Communications Personnel:
- Contact the:
a. Deputy Chiefs
b. Administrative Battalion Chief for Safety
c. City Safety Officer
d. On duty Battalion Chiefs, if not already contacted
e. Safety Lieutenant - Perform an administrative page
Procedure for reporting by Administrative Battalion Chief for Safety and Deputy Chiefs:
- Contact additional agencies as needed in accordance with the guidelines set forth in this document
- Contact the City Safety Officer and request that s/he make the appropriate L&I or OSHA notification
- If the City Safety Officer is not able to make the notification within the required timeframe, contact the local L&I office at 253-596-3800 or OSHA at 1-800-321-6742 within eight hours of the accident,
- Provide the following information:
a. Your name and “Tacoma Fire Department”
b. Location and time of the accident
c. Number of fatalities or hospitalized employees
d. Name and phone number of a contact person for TFD
e. A brief description of the accident
