Caudate nucleus Putamen Globus pallidus Subthalamic nucleus Substantia nigra
Striatum = caudate nucleus + putamen Lentiform = putamen + globus pallidus
lentiform is triangle shaped
What communicates between the two hemispheres?
Corpus callosum
Hyper vs hypodense on CT scan - stroke
Hyper = haemorraghe Hypo = ischaemic
What kind of brain bleed is assoc. with trauma and collapse then a lucid period followed by a further collapse?
Extradural
If ischaemic stroke what is your management?
As soon as haemoraghic is ruled out - ASPIRIN 300MG
<4.5hrs - thrombolysis with alteplase (condradicted in very high BP)
>4.5hrs - give 300mg of aspirin for 2 weeks then 75mg clopidogrel
If large vessel thrombus on CT = thrombectomy
What is the acronym to remember chronic stroke management?
HALTSS
HTN
Antiplatetelet/anticoagulation - after aspirin for 2 weeks then clopidogrel once daily OR AC with warfarin/rituaxaban if AF
Lipid lowering therapy - statin
Tobacco - smoking cessation
Sugar - screen and manage diabetes
Surgery - patients with ipsilateral carotid artery stenosis of 70-90% should be referred for carotid endarterectomy
What investigation is most important when monitoring a patient wit Guillan-Barre syndrome?
Vital capacity - can cause respiratory failure
