Secondary dementia due to ischaemia
Aetiology?
Thromboembolic or hypertensive cerebral infarcts of s/m vessels in cortex and white matter - causing cognitive impairment.
3 subtypes?
Stroke-related
Subcortical VaD - small vessel disease/Binswanger disease
Mixed - AZD & VaD
Risk Factors?
ϖ Stroke/TIA history ϖ Atrial fibrillation ϖ Hypertension ϖ Diabetes ϖ Hyperlipidaemia ϖ Smoking ϖ Obesity ϖ Coronary heart disease ϖ FM of CV disease
Core clinical features? (6)
Emotional and personality change - early sign
Cognitive deficits - incl memory , nocturnal confusion
Anxiety
Depressive episodes
Labile & confused episodes
Seizures
Style of disease progress?
Stepwise deterioration and intervening stabillity
Mx?
Identify cause and treat accordingly
What medications are not recommended by NICE?
ACh-i
Prognosis?
3-5 years
What medication should not be given if already on statin?
Beta blockers - rhabdomyolysis
