inflammation of the renal pelvic and parenchyma
what type of forms are there
acute and chronic
Aetiology of pyelonephritis
risks are the same as lower UTI
- e.coli
Risks of pyelonephritis
suppressed immune response
catheterization
urine reflux
diabetes (just a higher risk for infections in general)
Patho of pyelonenephritis
progresses in an ascending order
inflammation - tissue damage- formation of collagen depositions (fibrosis and scar formation)
what happens if the infection gets into the circulation
causes septicemia
what does fibrosis/scar formation cause
decrease in functional tissue therefore a decrease in renal function
What is the patho of the chronic form
recurrent inflammation causes urinary reflux/ obstruction
renal damage leads to renal failure
Manifestations of Upper UTI
acute onset lower back pain fever dysuria frequency urgency pyuria
what is a complication of Upper UTI!
severe HTN
Treatment
Ab (10-14 days long d/t advanced bacterium
- may consider IV Ab
