Aetiology of PID
Polymicrobial pyogenic (pus producing bacteria) -chlamydia, gonococci, staphylococci, streptococci untreated bacterial infections -10% gon 20% chlam
Patho of PID
Microbe enter the cervix when it is dilated during menstruation
rapid proliferation occurs as endometrium sloughs
it is an ascending infection
What is a common complication that arises from PID
Pelvic abscesses occur that may lead to peritonitis
Manifestations of PID
lower abdominal pain heavy purulent vaginal discharge adnexal tenderness dyspareunia bleeding fever
what are you at risk for with PID
leukocytosis (elevated white blood cells)
and
infertility (d/t scar tissue formation, fibrosis)
Diagnosis of PID
presentation is important (tenderness, discharge, ^WBC)
an elevated ESR (erythrocyte sediment rate - when antibodies are present E will cluster together)
an elevated CRP (another inflammatory protein)
Laparoscopy (to take a look)
Treatment of PID
AB (multiple broad spectrum ABs)
evaluate and treat partner (if an STD bacteria)
surgery: remove abscesses and scar tissue (reduce the risk for complications to arise)
