severity - mild
< 4 stools/day
only small amount of blood
severity - moderate
4-6 stools per day
varying amount of blood
no systemic upset
severity - severe
> 6 bloody stools per day
features of systemic upset
inducing remission - mild/moderate: proctitis
- topical (rectal) aminosalicylate
- if no remission 4wks add oral aminosalicylate
- then add oral corticosteroid
inducing remission - mild/moderate: proctosigmoiditis + L sided UC
- topical (rectal) aminosalicylate
- add high dose oral aminosalicylate or switch to high dose oral amisosalicylate + topical corticisteroid
- oral aminosalicylate and oral corticosteroid
inducing remission - mild/moderate: extensive disease
- topical aminisalicylate + high dose oral aminosalicylate
2. high dose oral aminosalicylate and oral corticosteroid
inducing remission - severe colitis
hospital treatment
IV steroids
if no improvement 72hrs consider adding IV ciclosporin to IV steroids
Maintaining remission: proctitis + proctosigmoiditis
topical aminosalicylate or oral aminosalicylate + topical aminosalicylate or oral aminosalicylate
Maintaining remission - L sided and extensive UC
low maintenance dose of oral aminosalicylate
maintaining remission after severe relapse or ≥2 exacerbations in past year
oral azathioprine or oral mercaptopurine
