cont inflammation limited to colon + rectum only superficial mucosa smoking is protective excrete blood+mucous use aminosalicytes primary scelrosing cholangitis
presentation
diarrhoea
abdo pain
passing blood
weight loss
testing
routine bloods
CRP
faecal calprotectin
endoscopy with biopsy
diagnosis using
endoscopy with biopsy
Crohns Mx: inducing remission
- steroids
if they dont work consider immunosuppresent
- azathioprine
- mercaptopuring
- methotrexate
Crohns Mx: maintainign remission
- azathioprine, mercaptopurine
alternatives
- methotrexate
- infliximab
Crohns Mx: surgery
if disease only affects distal ileum can resect it
can be used to treat secondary strictures and fistulas
UC Mx: inducing remission in mild-mod disease
- aminosalicylate
2. corticosteroid
UC Mx: inducing remission in severe disease
- IV corticosteroids
2. IV ciclosporin
UC Mx: maintaining remission
aminosalicylate
azathioprine
mercaptopurine
UC Mx - surgery
can remove colon + rectum (panproctocolectomy) and leave ileostomy or Jpouch
UC flare up: severe
passing >6 stool per day containing blood
systemic disturbance e.g. fever
