Generic Name: ipratropium
Trade Name: Atrovent
Classification: Synthetic quaternary ammonium compound (Anticholinergic, bronchodilator)
Medication Purpose: Inhibits interaction of acetylcholine at receptor sites on the bronchial smooth muscle, resulting in decreased cyclic guanosine monophosphate and bronchodilatation. Used for bronchospasm, COPD, rhinorrhea (nasal spray)
Route: INH
Side Effects: bronchospasm
Metabolized: Liver, minimal
Contraindications: Hypersensitivity to this product, atropine, bromide, soybean, or peanut products
Nsg Considerations: Monitor respiratory function, monitor for evidence of allergic reactions, paradoxical bronchospasm (withhold dose and call prescriber)
Health Teaching: Advise pt. not to use OTC medications unless approved be prescriber (extra stimulation may occur), to use this medication before other medications and allow at least 5 mins. between each to prevent over-stimulation. Instruct geriatric pts. to use spacer device, teach pt. the proper use of the inhaler, teach pt. if paradoxical bronchospasm occurs to stop product immediately. Limit caffeine products (chocolate, coffee, tea and colas.
