Generic Name: fluticasone
Trade Name: Flovent Diskus
Classification: Corticosteroid, inhalation, antiasthmatic
Medication Purpose: Decreases inflammation by inhibiting mast cells, macrophages, and leukotrienes; antiinflammatory and vasoconstrictor properties. Used for prevention of chronic asthma during maintenance treatment in those requiring oral corticosteroids; nasal symptoms of seasonal perennial and allergic/nonallergic rhinitis.
Route: INH
Side Effects: Eosinophilic conditions, angioedema, Churg-Strauss syndrome, bronchospasm.
Metabolized: In liver after absorption in lung
Contraindications: Hypersensitivity to this product or milk protein, primary treatment in status asthmaticus, acute bronchospasm.
Nsg. Considerations: Assess respiratory status, assess for withdrawal symptoms from oral corticosteroids (depression in joints, pain in joints, fatigue), monitor adrenal insufficiency, monitor growth rate in children; blood glucose, serum potassium in all pts.
Health Teaching: Teach pt. to report immediately cushingoid symptoms (no appetite, nausea, weakness, fatigue, decreased BP), teach pt. how to use and when it may be empty, advise pt. to use bronchodilator first before using inhalation (if using both). Caution pt. not to use for acute asthmatic attack (may require oral corticosteroids), advise pt. to avoid smoking, smoke-filled rooms, those with URI’s, those not immunized against chickenpox or measles.
