Generic Name: atorvastatin
Trade Name: Lipitor
Classification: HMG- CoA reductase inhibitor (Antilipidemic)
Medication Purpose: Reduces cholesterol synthesis, high doses lead to plaque regression. Used as an adjunct in primary hyperscholesterolemia, dysbetalipoproteinemia, elevated triglyceride levels; prevention of cardiovascular disease by reduction of heart risk in those with mildly elevated cholesterol.
Route: PO
Side Effects: Liver dysfunction, pancreatis, rhabdomyolysis
Metabolized: Liver
Contraindications: Pregnancy, breastfeeding, hypersensitivity, active liver disease
Nsg. Considerations: Assess nutrition, monitor triglycerides, cholesterol at baseline and throughout treatment, monitor bowel pattern daily (diarrhea), monitor liver function, monitor renal studies, assess eye via ophthalmic exam 1 mo. after treatment begins, assess for muscle pain, identify if pregnancy is planned (do not breastfeed).
Health Teaching: Teach pt. that risk factors should be decreased (high-fat diet, smoking, alcohol consumption), advise pt. that treatment will take several years, blood work and eye exam will be necessary during treatment, advise pt. to stay out of the sun, use protective clothing (photo-sensitivity may occur).
