Generic Name: QUEtiapine
Trade Name: SEROquel, Seroquel XR
Classification: Dibenzodiazepine (Antipsychotic)
Medication Purpose: Functions as an antagonist at multiple neurotransmitter receptors in the brain. Outcome: decreased hallucinations and disorganized thought.
Route: PO
Serious Side Effects: Seizures, neuroleptic malignant syndrome, tachycardia, QT prolongation, cardiomyopathy, myocarditis, leukopenia, agranulocytosis, rhabdomyolysis, Stevens- Johnson syndrome, anaphylaxis
Metabolism: Liver
Contraindications: Hypersensitivity, breastfeeding

Nsg. Considerations: Give reduced dosage in geriatric pts, avoid use of CNS depressants, give immediate release w/out regard to meals, give extended release w/out food or with light meal, do not split, crush, chew. Supervise ambulation (fainting is possible). Assess CV status, assess mental status before initial administration, not to be used in child <10 yr (immediate release) or <18 yr (extended release). Obtain baselines in blood glucose, liver function tests, neuro status, ophthalmologic exam, weight, throid function tests, serum electrolytes/creatinine/lipid profile/prolactin. Monitor BP, assess for EPS.
Health Teaching: Teach pt. to use good oral hygiene, inform pt. that orthostatic hypotension occurs often (avoid hot tubs, hot showers, tub baths. Advise pt. to avoid abrupt withdrawal, notify provider of thoughts of suicide.
