Trade Name: Tylenol
Classification: Nonsalicylate paraaminophenol derivative (Nonopioid analgesic)
Medication Purpose: May block pain impulses peripherally that occur in response to inhibition of prostaglandin synthesis. Outcome: decreased pain, fever
Route: PO, RECT, IV
Serious Side Effects: hepatotoxicity, hepatic seizure (O.D), GI bleeding, renal failure, leukopenia, neutropenia, hemolytic anemia (long-term use), thrombocytopenia, pancytopenia, Stevens- Johnson syndrome, toxic epidermal necrolysis, cyanosis, anemia, jaundice, CNS stimulation, delirium followed by vascular collapse, seizures, coma, death
Metabolism: Liver
Contraindications: Hypersensitivity to this product or phenacetin.
Nsg. Considerations: Monitor liver function studies before therapy if long-term use is anticipated, monitor renal function studies, blood studies, check I&O ratio, assess for fever and pain, chronic poisoning, assess hepatotoxicity.
Health Teaching: Inform pt. that toxicity may occur when used with other combination products, advise pt. not to use with alcohol, OTC products, or herbals w/out prescriber approval. Teach pt. to recognize signs of chronic overdose, inform pt. that urine may become dark brown as a result of phenacetin.
