Generic Name: sodium bicarbonate
Trade Name: Sellymin
Classification: NaHCO3 (Alkalinizer; antacid)
Medication Purpose: Orally Neutralizes gastric acid, which forms water, NaCl, CO2; increases plasma bicarbonate, which buffers H+ ion concentration; reverses acidosis. Used for acidosis (metabolic), cardiac arrest alkalinization (systemic/urinary); antacid (PO); salicylate poisoning.
Route: PO, IV
Side Effects: Tetany, seizures, cardiac arrest
Metabolized: Unknown
Contraindications: Respiratory/metabolic alkalosis, hypochloremia, hypocalcemia
Nsg. Considerations: Asses respiratory and pulse rate, rhythm, depth, lung sounds, assess for CO2 in GI tract (may lead to perforation if ulcer is severe), fluid balance, monitor electrolytes, blood pH, PO2, HCO3 (beginning treatment), monitor extravasation with IV admin. Alkalosis: irritability, confusion, twitching, hyperreflexia, stimulation, slow respirations, cyanosis, irregular pulse. Milk-alkali syndrome: confusion, headache, nausea/vomiting, anorexia, urinary stones, hypercalcemia.
Health Teaching: Instruct pt. to chew antacid tab and drink 8 oz. of water, do not take with milk (because milk-alkali syndrome may result), do not use antacid for more than 2 wks. Advise pt. to notify prescriber if indigestion is accompanied by chest pain; trouble breathing; diarrhea; dark/tarry stools, vomit that looks like coffee grounds, swelling of feet/ankles. Teach pt. about sodium-restricted diet; to avoid use of baking soda for indigestion.
