Posterior ciruclation stroke
seems to be very vague symptoms so diffcicult to diagnose can often be misidaignosed as a migraine since headache and dizziness is common esp in people known to have a history of migraines its easy to assume
- headach
- dizziness
can also present with other features like the d’s dysarhtria, dysphagia, motor, sendory deficits however small ones may only present with just vertigo
Neuro exam questions (clinical attachment) if suspect stroke
FAST SIGNS
any double vision, vision loss !

- squeeze my finger as hard as you can
- does it feel the same on each side
- power /5
- EYES , visual field and follow H and light eyes flash
- pronator drift
TIA what is it
tempparyr disruption of blood to the brain but resolves on its own, symtoms tyically last less than 24 h
if you suspect TIA whats the treatment
need to be on prophyactic to prevent another one
- aspirin (antiplatellet)
- life style modifciation
what does hints exam stand for
head impulse
nystagmus
test of skew
normal HI could mean
normal
or central cause of vertigo
so we do the head impluse test why
to check for peripheral caused os vertigo like labynthitis or vestibular
what is the indication for perfoing hints
pt comes in with dizziness that is constant and have nystagmus at rest with no central feautres/abnormal neurological findings
questions to ask to rule out a central cause of vertigo
ask for central features
headache or neck pain
the dangerous D’s
dysarthria
dysphonia
dysmetria
diplopia
gait issues
vertical nystagmus at rest
central and peripheral cause of vertigo
central - POSTERIOR CIRCULATION STROKE
PERIPHERAL - vn, labrynthitis
blood supply of posterioc circulation
brainstem and cerebellum !
