stasis derm
r/t chronic venous incompetency to lower legs secondary to chronic venous hypertension, may lead to ulcers, common cause of id reaction eczema
epidemiology
older patients, more common in women, due to incompetent valves, pooling blood, fluid and proteins leak into tissue
clinical features
pitting edema on ankle, calf, shin, pruritus, inflammation –> erythema and scaling, oozing, hemosiderian staining from extravation of RBCs,atrophie blanche (white scar tissue)
acute phase
infraepithelial edema with accumulation of macro and microvesicles, lymphocytes in the superficial dermis. in chronic – no inflammation. in both: dilated capilarries, hemosiderin, fibrosis
ddx
plaque psoriasis, irritant derm, contact derm, asteotic eczema, MF, tinea pedis
tx
control venous htn, compression stockings 20-30mmHg daily, exercise focused on calf muscles, triamcinolone 0.1% ointment at night, emollients, elevate legs above heart
