Lichen Planus
idiopathic skin disease with inflammatory features of an autoimmune nature involving T cells
Locations of LP
skin, hair, nails, genitalia, mucus membranes
LP of oral mucosa has increased risk of
SCC
Pathogenesis
healthy adults w no known cause, can be correlated with viruses, meds, allergens.
Clinical features
pruritic, planar, polyangular, purple, papule, plentiful… flat topped with uneven border with a violaceous and shiny hue
Wickams Striae
lacy white surface pattern in buccal mucosa
Koebner phenomena
rash induced by skin trauma such as surgery or scratching
scalp LP
Fibrosing alopecia with typical LP symptoms during inflammatory stage
Oral LP
With or without cutaneous symptoms. White is asymptomatic, red is erosive and presents with pain and swelling
Genital LP
vulvar scarring in erosive forms and burning and itching pain and sexual function impairment
Nail LP
Nail dystrophy or nail loss with itching and burning

Ddx
lichenoid drug eruption (ace, thiazide diuretics, antimalarials), graft v host, autoimmune blistering diseases
tx
accelerate resolution, most resolve within 1-2 years, prevent scarring, high potency topical steroids, intralesional steroids for hypertrophic LP, extensive can do systemic for 4-6 weeks, antihistamines, phototherapy
newly dx LP need this examined
