Epidemiology
more prevalent in caucasian men with significant photo damage > 40 years of age. Peaks in winter and the summer
Triggers for Grovers Disease
heat, sweat, friction, UV radiation
Pt with this condition are more likely to have Grovers Disease
asteatotic eczema
Clinical features
pruritic or asymptomatic papules, vesicles, photodistribution on chest, back, and thighs
Dermatopathology
acantholysis (loss of the dermasomes that hold keratinocytes together)
ddx
darier and hailey hailey disease are seen in younger individuals and have a family hx. PF will be seen via direct immunoflorescence, Grovers would not be remarkable
Treatment
control itch, avoid sweating and heat, topical corticosteroids/antipruritics, vit D analogues
Systemic adverse effects of systemic corticosteroids
Growth retardation, hypothalamopituitary adrenal axis suppression, Cushings
S/e topical steroids
atrophy, striae, hypopigment
Elidel and Protopic
decrease pruritus by blocking Tcells and cytokines. Good for steroid sparing areas. Can have transient skin irritation
Calcipotriene adverse effects
hypercalcemia, irritation, photosensitivity, allergic contact derm
Isotretinoin patho
decreases inflammation and the size of the pilosebaceous duct. Isotretinoin sometimes used for persistent grovers
