when should sepsis be suspercted?
wheree there is known or suspected infelction plus or minus risk factors for sepsis or sirs criteria and a systolic BP less than 100
what are the fluid doses?
elderly-chroc renal failure/hx cardiac failure - 500ml
all other 1000ml
what to do is there is a transport time greater than 30 minutes?
consult for 2g IV ceftriaxone
what are the sings symptoms suggestive of infection?
fever, chills, rigors, malaise
neurological: altered mental state, heachage, meneigitis, deteroation in ADLS
resp - cough and dysponea
abdominal- ridgidity, pain, tenderness, guarding, swelling
genitourinary- dysuria, frequency, heamtoyruia
skin- celulitis, petechail ras septic arthristis
infected wound
what are the risk factors for sepsis?
neutropenia, recent chemotherapy, other immunocompromised
recent pneumonia or COPD
recent trauma/surgury
indwelling medical devices
what is included in the SIRS criteria?
<36 or >38 temp
hr >90
RR >20
