what are the care objectivies?
pt hx alone is enough to suspect envenomation
efective PBI if required - limit pt movmemnt (bandage and splint
transport pt urgently to appropriate destination - consider consulting adult retreval or PIPER
what are the signs of systemic snake envenomation?
eyelid drooping, diplopia, slurred speach, drooling, generalised musle wakness - pain, are or lymph nods, headache abdo pain
nausa/vomitting, sweating
resp distress
LOC, paralysis
bleeding- from site, nose, gums, dark or red urine
how should the pressure bandage be applied?
should be as tight as for sprained ankle
endure doesnt cut of periperhal pulses - use 15cm broad elasticated roller bandage
what should be considered for a blue ringed octupus bite?
prolonged resus
due to paralysis pt suffer from resp arrest prior to cardiac arrest - hypoxia- with appropriate vent support prognisis is good
what should be considered for a tick bite?
may cause anyphalxis or slow developing paralysis over days
dont attmept to remove tick
what should you do if the transport time after a snake bite is over 30 mins?
consult ARV to facilitate transport to appropriate facility with anti-venom
what advice might be given for the administration of anti-venom?
maintain BLS and PBI
closly monitor vitals
IV acess 2 optimal- TKVO fluids and prepare avrenaline incase anaphylaxisis
draw up anti-venom and add it to a bag og normal saline 500ml
admin full 500ml over 20-30 mins
what general care should be given for all envenomation?
manage airway, ventialtion and perfusion
manage pain
manage nausea and vomitting
manage seizures
what is the managment for all australian skanes?
limb- pressure bandage, splint and immoblisation on the strecher
torso- immpbalise pt on strecher
IV acess times 2
manage resp distress with oxygen and supportive ventilations
what is the managment for redback spiders?
No PBI
apply ice pack to the site
what is the treatment for big black spiders?
dont walk pt
immobalise with PBI and splint and on strecher
manage pulmonary odeam with CPAP
what to do for non-tropical jellyfish - incl bluebottles?
wash effected area with sea water if possible
gently pick off remaining tenticles
apply warm/hot water to site if avaliable - consider icepack to site if not
do not apply vinager
what to do for barbed fish/rays?
dont attempt to remove the barb
mannage hemorrage
immersse in hot/warm water if avaliable
what to do for a blue rigned octupus?
limb - pbi and immobalise on strecher
torso- immobalise on strecher
prepare for deterioriation
prolonged resus efforts considered
