MRI changes
Plus clinical suspicion
New York criteria
- low back pain for at least 3 months improved by exercise and not relived by rest
- Limitation of lumbar spine motion in Sagitta and frontal plates
- Chest expansion decreases relative to normal values for age and sex
4a. Unilateral sacroiliitis grade 3-4
4b. Bilateral sacroiliitis grade 2-4
One of 1-3 then 4

Sacroiliitis on imaging and >1SpA feature
HLA- B27 positive and 2 other spA features
Sacroiliitis on imaging
Active inflammation on MRI highly suggestive of sacroiliitis associated with SpA
