Sepsis Mathiah Flashcards
Does Sepsis always present with a positive blood culture
No! Not all of the time
What is Sepsis
SIRS due to infection…Systemic inflammatory Response Syndrome
Leading cause of death in critically ill pts
true
Rate of sepsis by what organisms is increasing>
Candida….fungal organisms
Sepsis means what
Putrefaction (rot)
What are the SIRS criteria
temp over 38
heart rate over 90
RR over 20
White count over 12,000, less than 4,000 or over 10% bands.
Remember that some patients do not become febrile
Like the elderly or those with uremia (kidney disorder)…they may actually get colder
Afebrile infections affect who
- Extremes of Age
- Chronic Kidney Disease
- Corticosteroid Use
- NSAID/Acetaminophen use
- Diabetes
- Neurologic insults (stroke or brain malformation)
Severe sepsis
Sepsis plus organ dysfunction or hypotension/ hypotension (which causes the organ dysfunction
septic shock
sepsis that persists despite adequate fluid resuscitation
Inflamatory cytokines are
IL-1,6,8, TNF-Alpha……NEED TO KNOW THIS…COMMON KNOWLEDGE
ANti-inflammatory
IL-10
clinical manidestations
fever, tachycardia, tachypnea, hypotension, mental status change, nausea/vomitting
“Silver Day” of sepsis
Early treatment is crucial
Before antibiotics
get cultures…..gram stain and culture.
Two blood cultures: One peripherally and one through a vascular access device when present so that if the source of infection is the vascular device you will know.
