Strep Pneumo, Mycoplasma, Chlamydia pneumoniae, Haemophillus Influenzae
Staph Aureus
Think about it when you hear about a CA pneumoniae that is not getting better. Has PVL gene. Usually affects young adults. Multi-lobar.
Think viral pneumonia when
It seems to be a seasonal outbreak or when a lot of ppl get it at once. Influenza, H1N1
Most common route to lung
Aspiration is most common (we aspirate just a little almost every night), inhalation
Pneumonia most often occurs when natural defenses are impaired such as coughing (paralysis), Sneezing (stroke), mucociliary escalator (smoking)

Findings
Fever, Tachypnea (resp rate over 25), tachycardia can be a sign. Heart rate over 100, fever over 37.8, and RR over 20 inc the likelihood by 3 in one study. And of course the proper findings on lung exam
BRONCHIAL SOUNDS—Loud airway like sounds heard in the lungs. Normally heard over large bronchi
Two main tools to grade severity of pneumonia
PSI : Pneumoniae severity index- depends on age, comorbidities, sickness on presentation. Points are assigned and you classify based on point total.
Curb- 65: Consciousness, Urea, Resp Rate over 30 Blood Pressure less than 90, Age over 65
