What is the microbiology of Yersinia Pestis
Chubby Gram negative rods (safety pin appearance)
What does the F1 virulence factor do?
It paralyzes phagocytes upon ingestion
What is the most common form of the plague?
Bubonic plague
Pneumonia that follows the bubonic form is called?
Secondary plague pneumonia
How do you become infected with primary plague pneumonia>
Infected person or cat, suspect bioterrorism in this situation
What percent of bubonic plague cases are fatal?
60%. Bubonic plague is attained from a flea bite and causes swollen painful inguinal nodes.
Besides the Bubonic form, what other diseases can YP cause?
Septicemia- may or may not have buboes, petechiae and DIC, will have vomitting and diarrhea
Pneumonic- YOU WILL DIE IF YOU AREN”T TREATED. Requires isolation and prophylaxis for all exposed
What is the character of the sputum production in the pneumonic form of YP infection?
Bloody/watery
How would you differentiate between Tularemia and Bubonic plague
The inoculation site would look much worse in Tularemia. Also Tularemia would have a non-productive cough where YP would have a productive cough with bloody watery sputum.
Diagnosing pneumonic plague
Find antibody for FI toxin, history of exposure, Bipolar stain (looks like safety pin, each end stains but the middle does not).
What are teh 3 zoonoses?
Yersinia Pestis, Brucella Spp, Coxiela Burnetti
What is the reservoir for Brucella
Cows, sheep, pigs
How does one get Brucella
contact with an infected animal or unpasteurized cheese
Those at risk
Vets/ farmers, recent immigrants from Mexico or mediterranean
Characteristics of Brucella infection
slow loving, chronic, undulating fever/night sweats
May have acute onset with fever or flu symptoms
Lower vertebrae pain, or heart/liver infection
Granuloma formation on the bone or liver
