sarcoidosis Flashcards
what is Sarcoidosis
a chronic granulomatous disorder characterised by accumulation of lymphocytes and macrophages in the lungs and other organs.
who is it most common in
YP and afrocaribbean
who is it less common in
smokers
which type of hypersensitivity reaction
IV
acute CF
- Bilateral hilar lymphadenopathy
- Erythema nodosum
- Non caseating granulomas
- Fatigue
- Uveitis
- Weight loss
- Myocarditis
- Neuropathy
chronic CF
hypercalcaemia
skin infiltration
lung infiltrates
peripheral lympadenopathy
what do lung infiltrates cause
alveolitis
why do you get hypercalcaemia
macrophages inside granulomas cause increased conversion of vitamin D to its active form
what will pulmonary function test show
restrictive pattern with reduced transfer factor
what does transfer factor represent
CO diffusing capacity corrected for alveolar volume
what is TF low in
emphysema and interstitial lung diseases
CXR
bilateral hilar lymphadenopathy
CT
peripheral nodule infiltrate
what are some causes of BHL
EAA
infection eg TB
organic dust disease eg silicosis and berylliosis
blood
inc serum ACE
inc Ca
inc inflammatory markers
