Renal replacement therapy Flashcards
When is dialysis normally considered?
eGFR <15 and patient symptomatic
What are the 2 main methods of dialysis?
Haemodialysis
Peritoneal dialysis
What are some advantages of haemodialysis? 2
Effective
3-4 dialysis free days a week
What are some disadvantages of haemodialysis? 2
In hospital several days a week for a few hours at a time
Diet and fluid restriction
What are some complications of haemodialysis? 4
Intra-dialytic hypotension
Arrythmia
Clotting of vascular access
Infection
What are the advantages of peritoneal dialysis?
More independence
Fewer restrictions on diet and fluid
What are the disadvantages of peritoneal dialysis?
Daily
Have to receive training
What are the complications of peritoneal dialysis?
Exit site infection Peritonitis Ultrafiltration failure Encapsulating peritoneal sclerosis Hypotension Hernia
What can cause peritonitis in peritoneal dialysis?
Gram + = skin
Gram - = bowel
What are the 2 types of donor?
Live donor
Cadaveric
What are some contraindications to transplant?
Cancer Cardiac dysfunction Recurring conditions HIV/AIDS Frailty BMI >35 Non compliance
What is the induction immunosuppression post transplant?
Basoliximab
What is the maintenance immunosuppression post transplant?
Tacrolimus + mycophenolate + prednisolone
What are the types of rejection in transplant?
Hyperacute- rare
Acute
Chronic
What is hyper acute rejection?
Within minutes of transplant
Donor specific antibodies lead to severe necrosis and thrombosis
