What is acute tubular necrosis?
Necrosis of tubular epithelial cells
What is the histology of acute tubular necrosis?
Tubular epithelium necrosis, dilatation of tubules, and necrotic cells obstructing lumen
What are the causes of acute tubular necrosis?
Toxic
Ischaemic
What are the toxic causes of acute tubular necrosis?
Drugs- esp gentamicin
Myoglobin- secondary to rhabdomyolysis
Radiocontrast
What investigations are done for acute tubular necrosis?
Muddy brown casts in urine= pathognomonic
Raised urea, creatinine and potassium
What is the most common renal cause of AKI?
Acute tubular necrosis
What is the management of acute tubular necrosis?
Treat cause
Supportive
What is the prognosis for acute tubular necrosis?
Reversible in early stages
What is the presentation of acute tubular necrosis?
Hypotensive, tachycardia
Generally unwell
Oliguria, anuria
Dark coloured urine
