Exaggerated skin markings
Dry scaly surface

Scratch marks

Lichen simplex
Itchy erosions and/or scales between toes
Scaly erythematous ring with central sparing

Tinea pedis/other
Clusters

Pemphigus vulgaris
Symmetrical on scalp, shoulders, buttocks, elbows and knees
Itchy papules or vesicles
Eroded/crusted due to scratching
History of coeliac disease

Dermatitis herpetiformis
Localised, blistering, painful rash
Unilateral
Initially pain without skin change
Crust
Dermatomal distribution

Shingles (Herpes Zoster)
Tender bilateral erythematous subcutaneous nodules
Ill-defined, warm, oval, round or arciform, and without ulceration
3-20cm diameter

Erythema nodosum
Red, expanding patch of skin
Bull’s eye appearance
Mostly asymptomatic
At area of tick bite

Erythema migrans
Rapidly enlarging, very painful ulcer
Full thickness ulcer
Blue undermined borders
Pathergy (areas of local trauma)
Assoc with autoimmune disease

Pyoderma gangrenosum
Severe itch worse at night
Trunk and limbs, spares scalp
Can see burrows
Rash of varied appearance

Scabies
Single plaque appears before rest of rash (herald patch)
Scaly patches/plaques mainly on chest and back
Secondary lesions smaller than herald patch
Sometimes follows URTI

Pityriasis rosea
Widespread sterile pustules
Background of red and tender skin

Generalised pustolosis/pustular psoriasis
