What are the CRB65 cut offs for management in community vs hospital?
0 - usually treat at home
1-2 - usually treat at home, consider hospital if major co-morbidity
3-4 - urgent hospital admission
What to do if someone has signs of bilateral pneumonia in community?
Hospital referral
What CURB65 score means severe pneumonia?
3-5
Management of mild CAP?
Oral Amoxicillin 5 days (doxycycline if pen allergic)
Management of severe CAP?
Co-amoxiclav IV and doxycycline PO 5 days
If pen allergic levofloxacin IV
What is the step-down treatment for all patients with severe CAP?
Doxycycline
What drug would you substitute doxycycline with if the patient cannot take meds orally?
Clarithromycin IV
Non severe HAP management?
Amoxicillin PO 5 days (doxycycline if allergic)
Severe HAP management?
IV Amoxicillin and gentamicin (co-trim and gentamicin if allergic) 7 days IV/PO total
Severe HAP step down?
Oral co-trim
Non-severe aspiration pneumonia management?
PO amoxicillin and metronidazole (doxycycline if allergic) 5 days
Severe aspiration pneumonia management?
IV amox, met and gent (if allergic replace amox with PO doxycycline or IV clarithromycin) 7 days total
Severe aspiration pneumonia step down?
PO amox and met (doxy if pen allergic)
